This survey has been designed and is being managed by Ignetica Ltd (Ignetica) on behalf of Dell Computer Corporation Ltd (Dell). This survey & associated competition is governed by the laws of England and Wales. The pan-European survey is being undertaken as part of a wider research programme by Ignetica for Dell to gauge the forecast demand and challenges associated with data and archiving storage in healthcare. All information provided will be treated in confidence and will be used exclusively for analysis purposes. Anonymised summary information will be published in due course.  No personally or corporately identifiable information will be included in this analysis and no such information will be released to any parties other than Ignetica or Dell. Participants email addresses are optionally requested in order that a summary of the survey findings can be emailed to participants and that they may also be included in the random prize draw. Provision of this information is entirely voluntary and it is possible to complete the survey without this information.  Any information provided will be kept securely in accordance with Ignetica’s privacy policy and will be used for this research only. Participation in this survey and associated prize draw is entirely voluntary.  

 

 

Terms for Prize Draw

 

By entering the competition all entrants are deemed to have accepted these terms and conditions. The decision of Ignetica and appointed representatives is final in all matters and no correspondence will be entered into.

 

1. The survey will close when 1000 responses have been received, or on 1st July 2011 if 1000 responses have not been received by this date. At this point entry into the prize draw will close.  In the event of delayed commencement or low response levels we reserve the right to extend the timescales at our sole discretion.

 

2. The competition is open to all Healthcare IT professionals aged 18 years or over excluding employees of Ignetica or Dell and their families, their agents, or anyone professionally connected with this competition.

 

3. Only one entry per person is permitted. Automated entries will be invalidated.

 

4. The three winning entries, to be drawn at random, will receive a Dell Streak 7” Android Tablet computer in accordance with the prize draw description.

 

5. The winner will be notified by e-mail within 28 days of the closing of the online survey. If the winner does not claim their prize within four weeks of notification the prize will be awarded to another participant.  The email will be generated by an automated e-shot system and it is each participant’s responsibility to ensure that this is receivable without being spam filtered.  Where receipt of the email is delayed or prevented by such systems we will not be liable for such delay.

 

6. There are no cash or other alternatives to the prize stated. Winners are responsible for all applicable taxes and expenses not specified in the prize description. In the event of circumstances beyond our control, we reserve the right to offer an alternative prize/s of equal or greater value. Dell will supply the prizes directly.

 

7. By entering the prize draw, entrants acknowledge that they may be required to be used in publicity without further consent or payment.  Such publicity may include without limitation the entrant’s name and/or likeness, voice, photographs, biographical information and/or any statement made by them concerning the prize draw and/or the prize(s).  Entrants agree that any or all such elements may be used by the Promoter in any and all media in perpetuity throughout the world as the Promoter in its discretion see fit.

 

8. The web survey may be suspended temporarily and without notice in the case of system failure, maintenance or repair or for reasons reasonably beyond the control of Ignetica. Ignetica will not be liable in the event that all or any part of the service relating to the competition is discontinued, modified or changed in any way.

 

 

Terms for Summary Results

 

Survey participants may opt to receive a summary report presenting the aggregate responses to this survey.  No personally or corporately identifiable information will be included in this report.

 

1. The report will be provided electronically as a PDF document.  A link to the PDF or a PDF document will be emailed to all participants who have opted to receive the report.

2. The email will be generated by an automated e-shot system and it is each recipient’s responsibility to ensure that this is receivable without being spam filtered.  Where receipt of the email is delayed or prevented by such systems we will not be liable for such delay.    

3. We anticipate the report being available within 8 weeks of the survey closing, but we reserve the right to extend these timescales at our sole discretion. The report will be available in English only.  Copyright in the report will rest with Dell/Ignetica.

 

This agreement is governed by the laws of England and Wales.
